Nanotechnology is the latest in a growing list of emerging technologies that includes nuclear technologies, genetics, reproductive biology, biotechnology, information technology, robotics, communication technologies, surveillance technologies, synthetic biology, and neuroscience. As was the case for many of the technologies that came before, a key question facing nanotechnology is what type of regulatory oversight is appropriate for this emerging technology. As two of us wrote several years ago, the question facing nanotechnology is not whether it will be regulated, but when and (...) how.Yet, appropriate regulation of nanotechnology will be challenging. The term “nanotechnology” incorporates a broad, diverse range of materials, technologies, and products, with an even greater spectrum of potential risks and benefits. This technology slashes across the jurisdiction of many existing regulatory statutes and regulatory agencies, and does so across the globe. Nanotechnology is developing at an enormously rapid rate, perhaps surpassing the capability of any potential regulatory framework to keep pace. Finally, the risks of nanotechnology remain largely unknown, both because of the multitude of variations in the technology and because of the limited applicability of traditional toxicological approaches such as structure-activity relationship to nanotechnology products. Risk management of nanotechnology is challenged by the enormous uncertainties about the risks, benefits, properties, and future direction of nanotechnology applications. Because of these uncertainties, traditional risk management principles such as acceptable risk, cost–benefit analysis, and feasibility are unworkable, as is the newest risk management principle, the precautionary principle. Yet, simply waiting for these uncertainties to be resolved before undertaking risk management efforts would not be prudent, in part because of the growing public concerns about nanotechnology driven by risk perception (...) heuristics such as affect and availability. A more reflexive, incremental, and cooperative risk management approach is required, which not only will help manage emerging risks from nanotechnology applications, but will also create a new risk management model for managing future emerging technologies. There is much we do not know about nanotechnology. Despite its tremendous promise, nanotechnology today is mostly forecast and fervent hope. Predictions that spending on nanotechnology will increase from current levels of $13 billion to more than $1 trillion by 2015 are no more than that – simply predictions. Hopes that nanotechnology will be an essential part of solving the globe's energy, food, and water problems should be tempered by recalling a century of revolutionary technologies that failed to live up (...) to their early promise such as nuclear energy, supersonic airplanes, or gene therapy. Many other questions continue to nip at nanotechnology's heels, not the least of which are debates about what is and is not technically feasible. Despite these uncertainties, we can have complete confidence in one aspect of nanotechnology's future – it will be subject to a host of regulations. (shrink)

Trust is a topic of longstanding philosophical interest. It is indispensable to every kind of coordinated human activity, from sport to scientific research. Even more, trust is necessary for the successful dissemination of knowledge, and by extension, for nearly any form of practical deliberation and planning. Without trust, we could achieve few of our goals and would know very little. Despite trust’s fundamental importance in human life, there is substantial philosophical disagreement about what trust is, and further, how trusting is (...) normatively constrained and best theorized about in relation to other things we value. The possibility of extended cognition invites the possibility of extended knowledge. We examine what is minimally required for such forms of technologically extended knowledge to arise and whether existing and future technologies can allow for such forms of epistemic extension. Answering in the positive, we explore some of the ensuing transformations in the ethical obligations and personal rights of the resulting ‘new humans.’.

The proclamation of the death of God came at a pivotal time in the history of humankind. It far transcended the concerns of the religious faithful and dented the entire fabric of human existence. Left to its own devices, humans intended their consciousness to replace God's. This proved to be a terrible mistake that collapsed the entire modern project. One of the worldviews that emerged in the wake of this eruption was antinatalism, which refers to the conviction that human reproduction (...) should be brought to an absolute halt. This is the most modern outgrowth of the death of God and represents the most radical face of secular humanism. In spite of the admittedly dark fumes that leak out from the term ‘antinatalism’, this philosophical position emerges quite naturally when we consider the depletion of our traditional sources of philosophical enquiry. Combinatory logic and lambda-conversion were originally devised in the 1920s for investigating the foundations of mathematics using the basic concept of 'operation' instead of 'set'. They have now developed into linguistic tools, useful in several branches of logic and computer science, especially in the study of programming languages. These notes form a simple introduction to the two topics, suitable for a reader who has no previous knowledge of combinatory logic, but has taken an undergraduate course in predicate calculus and recursive (...) functions. Contemporary debates about epistemic luck and its relation to knowledge have traditionally proceeded against a tacit background commitment to cognitive internalism, the thesis that cognitive processes play out inside the head. In particular, safety-based approaches reveal this commitment by taking for granted a traditional internalist construal of what I call the cognitive fixedness thesis—viz., the thesis that the cognitive process that is being employed in the actual world is always ‘held fixed’ when we go out to nearby possible worlds to (...) assess whether the target belief is lucky in a way that is incompatible with knowledge. However, for those inclined to replace cognitive internalism with the extended mind thesis, a very different, ‘active externalist’ version of the cognitive fixedness thesis becomes the relevant one for the purposes of assessing a belief’s safety. The aim here will be to develop this point in a way that draws out some of the important ramifications it has for how we think about safety, luck and knowledge. (shrink)

The Anderson-Friedman absolute objects program has been a favorite analysis of the substantive general covariance that supposedly characterizes Einstein's General Theory of Relativity (GTR). Absolute objects are the same locally in all models (modulo gauge freedom). Substantive general covariance is the lack of absolute objects. Several counterexamples have been proposed, however, including the Jones-Geroch dust and Torretti constant curvature spaces counterexamples. The Jones-Geroch dust case, ostensibly a false positive, is resolved by noting that holes in the dust in some models (...) ensure that no physically relevant nonvanishing timelike vector field exists there, so no absolute object exists. The Torretti constant curvature spaces case, allegedly a false negative, is resolved by testing an irreducible piece of the metric, the conformal metric density of weight -2/3, for absoluteness; this geometric object is absolute. A new counterexample is proposed involving the orthonormal tetrad said to be necessary to couple spinors to a curved metric. The threat of finding an absolute object in GTR + spinors is overcome by the use of an alternative spinor formalism that takes a symmetric square root of the metric (with the help of the matrix diag(-1,1,1,1)), eliminating 6 of the 16 tetrad components as irrelevant. The importance of eliminating irrelevant structures, as Anderson emphasized, is clear. The importance of the choice of physical fields is also evident. A new counterexample due to Robert Geroch and Domenico Giulini, however, finds an absolute object in vacuum GTR itself, namely the scalar density $g$ given by the metric components' determinant. Thus either the definition of absoluteness or its use to analyze GTR's substantive general covariance is flawed. Anderson's belief that all absolute objects are nonvariational (that is, not varied in a suitable action principle) and vice versa is also falsified by the Geroch-Giulini counterexample. However, it remains plausible that all nonvariational fields are absolute, so adding nonvariationality as a necessary condition for absoluteness, as Hiskes once suggested, would likely leave no useful work to the Anderson-Friedman condition of sameness in all models. Simply having only variational fields in an action principle (suitably free of irrelevant fields) might be a satisfactory analysis of substantive general covariance, if one exists. This proposal also resembles the suggestion that GTR is "already parameterized," if one decides to parameterize theories by defining the nonvariational fields in terms of preferred coordinates called clock fields. More questions need to be addressed. Which fields should be tested for absoluteness: only primitive fields (which ones?), or all or some (which?) of their concomitants also? Geroch observes that some kinds of geometric objects, such as tangent vectors, scalar densities, and tangent vector densities of non-unit weight, satisfy the condition of sameness in all models if they merely fail to vanish. If these "susceptible" geometric objects can hardly help being absolute, to what degree are they, or the theories harboring them, responsible for this absoluteness? The answer to this question helps to determine the significance of the Geroch-Giulini counterexample. (shrink)

Background: Omalizumab, an anti-immunoglobulin E antibody, reduces exacerbations and symptoms in uncontrolled allergic asthma. The study objective was to estimate the costs and consequences of omalizumab compared to usual care from a US payer perspective. Methods: We estimated payer costs, quality-adjusted survival (QALYs), and the incremental cost-effectiveness ratio (ICER) of omalizumab compared to usual care using a state-transition simulation model that included sensitivity analyses. Every 2 weeks, patients could transition between chronic asthma and exacerbation health states. The best available evidence (...) informed the clinical and cost input estimates. Five years of omalizumab treatment followed by usual care was assumed to estimate a lifetime horizon. Omalizumab responders (60.5% of treated) were modeled as a separate scenario where nonresponders reverted back to usual care after 16 weeks of active treatment. Results: The mean lifetime discounted costs and QALYs were $83 400 and 13.87 for usual care and $174 500 and 14.19 for omalizumab plus usual care resulting in $287 200/QALY (95% interval: $219 300, $557 900). The ICER was $172 300/QALY when comparing omalizumab to usual care in the responder scenario. One-way sensitivity analyses indicated that the results were sensitive to the difference in treatment-specific utilities for the chronic state, exacerbation-associated mortality, omalizumab price, exacerbation rates, and response definition. Conclusions: The results suggest that adding omalizumab to usual care improves QALYs at an increase in direct medical costs. The cost-effectiveness of omalizumab is similar to other chronic disease biologics. The value increases when omalizumab response is used to guide long-term treatment. (shrink)
